Structure of the statement: what it consists of

A classic statement is written in the name of an official or authority and has a strict framework. Adhering to this framework is half the success.

1. Header (top right corner). Here you indicate 'To' — the position, name of the authority, and, if possible, the surname of the leader (for example, to the head of the district TCK and SP). Below 'From' — your full surname, first name, patronymic, registration address or actual residence, and contact phone number. A clear header ensures that the document reaches the addressee.

2. Document title. In the center, write the word 'Application' (or 'Заява' in the Ukrainian version). Sometimes a brief subject is added: 'regarding the provision of a deferment'.

3. Main part. Briefly and substantively outline the factual circumstances: who you are, what your basis is, and how it is confirmed. Write without emotions, in a businesslike manner, and refer to the attached documents.

4. Request part. Starting from the paragraph with the word 'I request', formulate a specific demand: to provide a deferment, to consider a complaint, to calculate a payment. One request — one clear point.

5. Attachments. List the copies of documents indicating the number of pages. This confirms the completeness of the package.

6. Date and signature. At the bottom left — the date, on the right — personal signature with a full name. Without a signature and date, the document is considered incomplete.

What types of applications are there

The form of presentation is the same, but the content and recipient depend on the purpose of the appeal:

Application for deferment from mobilization. Submitted to the territorial recruitment center (TCK and SP). It specifies the grounds for deferment — caring for a relative, disability, having many children, education, and other categories provided by law — and attaches supporting documents.

Complaint about the actions of the TCK. If you believe the actions of the staff are unlawful, the complaint is submitted to a higher authority, the management of the TCK, or to the supervisory authorities. The text describes what exactly was violated, when, and by whom.

Application for benefits. Concerns social benefits, assistance, or financial support. The recipient is the relevant social protection authority, bank, or agency responsible for payments.

Application for IDP status. Internally displaced persons submit it to obtain or confirm a certificate of registration, indicating the actual place of residence.

What documents to attach

The set of attachments depends on the type of application, but the logic is the same: each statement in the text must be supported by a document. Most often required are:

a copy of the passport and identification code; military registration document (for appeals to the TCK); birth certificates of children, marriage or divorce certificates; medical conclusions and certificates of health status or disability; family composition certificate; documents on education or employment; IDP certificate — depending on the situation. Attach copies, and keep the originals with you to present upon request. Copies of signatures and seals must be legible.

Where and how to submit

The method of submission is just as important as the content, because it provides you with proof of the appeal. There are three reliable options.

In person through the office. Prepare two copies: submit one, and on the second, request a stamp of acceptance — entry number and date. Keep this copy.

By registered mail with a list of contents and a delivery notification. The list records what exactly you sent, and the notification confirms that the document has been received.

Through electronic services. Some applications are accepted online through official government portals and applications. Keep the confirmation of submission and the assigned number.

Regardless of the method, always keep proof of submission and, if possible, find out the established deadline for processing the application.

Frequently Asked Questions

Is it necessary to write the application by hand?
No. The application can be typed on a computer and printed, but the personal signature and date must be written by hand. A handwritten version is also acceptable if the text is legible.

What should I do if I don't have some of the documents?
Submit the application with the documents you have and indicate which certificates are in the process of being obtained. The missing papers can be submitted later, keeping the confirmation of the initial submission.

How can I tell that the application has been accepted?
When submitted in person — by the stamp with the incoming number on your copy, when sent by mail — by the delivery notification, online — by the application number in the system.

Can one application be submitted for several issues at once?
It is not recommended. It is better to prepare a separate document for each request addressed to its recipient — this way, the request is easier to consider and harder to 'lose' one of the requests.
